First Link Learning Series in Ottawa

Led by the Alzheimer Society of Ottawa and Renfrew County in collaboration with the Champlain Dementia Network, First Link® is a framework that links individuals newly diagnosed with dementia and their families to a community of learning, services and support. It offers information, support, referral to community support agencies and a progressive learning series. The learning series include: First Steps for People with Dementia and Caregivers, Next Steps for Families, Care Essentials, Options for Care, as well as Late Stage Dementia Care and End of Life Dementia Care.

Please review the following options for the series which is relevant to your circumstances. Contact any of our chapter’s Program staff at 613-523-4004 with your questions or to learn about the upcoming dates and locations for our various First Link sessions.

For Individuals & Families

First Steps for People with Dementia and Caregivers
This series is to help individuals newly diagnosed with a dementia and their caregivers understand their diagnosis and the changes they experience. This series will also offer participants an opportunity to share and connect with others embarking on a similar journey.

Prochaines étapes/Soins Essentiels
Ce programme de 3 sessions a pour objectif de donner des renseignements sur la maladie d’Alzheimer et des sujets connexes. Prochaines étapes est offert aux aidants naturels ou un membre-famille ainsi que la personne dans la phase initiale de la maladie d’Alzheimer ou d’une autre démence. Les participants doivent avoir reçu un diagnostique du médecin.
Next Steps for Families
For family caregivers of persons with dementia who are presently living at home or in a retirement residence. This 3-week series provides an introduction to Alzheimer’s disease and dementia.

Care Essentials
For family caregivers of persons with dementia who are presently living at home, in a retirement residence. This 3-week series will focus on the middle stage of Alzheimer’s disease and related dementia: day to day care approaches and understanding behaviour changes.

Options for Care
For family caregivers of persons with dementia at home or retirement residence. This 2 week series will focus on community care resources and long term care: how the system works.

Late Stage Dementia Care and End of Life Dementia Care
This 2-part learning session is for family caregivers of individuals with dementia living in a long term care home. Sessions are facilitated by health care professionals and staff of Champlain Dementia Network organizations.

Working together to link individuals and families
affected by Alzheimer's disease to a community of
learning, services and support
.

First Link is a coordinated education and support initiative of the Alzheimer Society of Ottawa and Renfrew County, in collaboration with the Champlain Dementia Network.

Family physicians, diagnosticians, social workers, nurses, occupational therapist, and other health care providers may refer clients directly to First Link.

Each family you refer will receive:
• a phone call from the First Link coordinator within 4 weeks
• an information package
• information about learning series and support programs
• regular mailings of First Link programs and services

Referring clients to First Link:
• ensures that families are connected early to education, support and services.
• helps to reduce overall incidence and intensity of caregiving crisis situations.
• helps to strengthen the link between diagnosing family physicians, the Champlain Dementia Network and community service providers
• helps to increase effective and efficient use of community resources

You can refer any client with Alzheimer's disease or a related dementia and their family. Referrals are accepted by phone, fax or email. Please contact the First Link Coordinator with:

Designed for family members, caregivers, and the public, seminars are held on topics relating to various aspects of caregiving. Topics such as: Capacity, Powers of Attorney, Financial and Estate Planning, communication, challenging behaviours, coping strategies, and ethics are presented. Resources for future reference are shared.
